Description

Part Time Care Specialist Services are provided off site at clients’ homes/ RRP sites or other appropriate locations in the community, specifically during medication monitoring windows on the weekends.

Duties and Responsibilities

1. Visits clients in their homes to provide medication monitoring services to ensure clients are taking medications as prescribed and other case management functions as needed.
2. With guidance from supervisor or Manager On-Call on weekends, implements crisis support/ intervention techniques as needed and facilitates appropriate utilization of community support system (i.e. Crisis Center).
3. Prepares medication monitoring documentation as required in a thorough, accurate and timely manner.
4. Completes timely, billable service notes properly in Electronic Health Record.
5. Accurately reports to supervisor and peers issues related to client progress, program needs, and issues related to the Care Specialist successfully completing job duties.
6. Monitors health and safety in the homes. Responsible for addressing immediate health/safety concerns and related issues to supervisor.
7. Provides transportation (in Montgomery Station vehicles if available or personal vehicle) when deemed necessary for assisting clients in meeting their needs.
8. Performs other duties as assigned by supervisors.

Hours 8 am - 10:30 am or 7 pm - 9:30 pm Saturdays & Sundays


Qualifications

Competencies: Organizational and time management. Ability to establish rapport and to assist client to identify his or her own needs (i.e. self-care skills, social skills, independent living skills, medication management, health- mental and somatic, entitlements, and vocational) to establish appropriate rehabilitation goals and to provide internal and/or community referrals when necessary.
Education: High school Diploma required. BA/BS in Human Service and/or related work experience preferred. Additional certifications/trainings required (or provided during orientation, or shortly thereafter):
Language and Communication Skills: Good oral and written communication.
Technical Skills: CPR/First Aid, Infection Control, Food Safety and Handling, and Crisis Intervention/Suicidal Protocol. A current, valid driver’s license, proof of auto insurance (which meets minimum MD state requirements), and a current driving record with 3 points or less are required for transporting consumers, either in agency vans or a personal vehicle.
